Even Correct Information Can Be Unsafe in the Wrong Context: Social Control for Healthcare AI
Social Physical AI - Part 9 of 13 If a medical AI gives factually correct information, is that enough? Not necessarily. Correct information can still be delivered to the wrong person, at the wrong time, without consent, with too much certainty, or in a situation that should have triggered professional intervention. Healthcare makes the social control problem especially visible. The system must control more than content A healthcare interaction may involve: - the person’s emotional and comprehension state - consent - confidentiality - relationships among patient, family, caregiver, and clinician - role and decision authority - urgency - escalation thresholds This means safety cannot be reduced to “generate an accurate answer.” The architecture needs control before and after generation. Pre-generation policy The LOGIHEART source material describes a pre-generation policy layer. Before the model produces a response, the system can determine constraints such as: - do not present uncertain information as a diagnosis - do not disclose this information to a third party without authorization - ask for confirmation when identity, consent, or intent is unclear - if risk crosses a defined threshold, stop normal conversation and escalate This changes the order of operations. Instead of asking only “What would the model say?”, the system first asks “What is this system allowed to do in this relationship and situation?” Post-generation inspection Even with a policy, generative models can produce unexpected output. A second layer can inspect the response for violations: - confidentiality breach - prohibited disclosure - excessive certainty - conflict with current consent - missing escalation - mismatch with the selected communication policy If the response fails, the system can suppress it, regenerate under stricter constraints, or hand off. Handoff thresholds should be designed, not improvised High-risk AI should not decide its own scope of authority dynamically without boundaries. The system needs predefined conditions under which a professional must take over. Human handoff is therefore not merely an error path. It is part of the normal architecture of responsible autonomy. Sociality becomes part of safety engineering In healthcare, social intelligence means more than empathy. It includes knowing: - who may receive information - what consent applies now - whether the person is able to process the response - when uncertainty requires clarification - when the AI must stop and transfer responsibility A correct answer is useful only when delivered inside the correct relationship and responsibility boundary.
